PADI SEAL TEAM SUMMER CAMP WAITLIST REGISTRATION

I understand I am registering my child for a waitlist, and that I need to bring my chld to the shop as soon as possible to get fitted. This registration does not require a deposit or payment until a spot is open, and I understand that an additional spot/camp session may not open. In the event a spot does not open, pleae contact me with future opportunities for my child. Camps may open for winter and spring breaks.

I undestand I must print and complete the above forms and bring them to the shop no later than August 5th, along with the camper in order to fit equipment and prepare for the camp. (Allow 20-30 minutes for th visit). 1741 Second Street Southwest in Rochester.

I agree that SSE may take and use photos of my child/camper I'm registerting above. The photos and videos may be used for promotional marketing purposes. Checking the box here is my express authorization and release to use the camp vidoe and photo for any purpose SSE chooses.
